Rainbow Hospice Care is a community based not-for-profit organization that provides a full spectrum of physical, emotional and spiritual care to terminally ill patients and their families in both in-home and inpatient settings.

Founded in 1990, Rainbow Hospice Care's dedicated caregivers have been serving residents in south-central Wisconsin for more than 25 years, supporting patients and families residing in Jefferson County, as well parts of Dane, Walworth, Dodge, Waukesha, Rock, and Columbus counties.

Rainbow Hospice Care's main office is located in Jefferson which is home base for most clinical and nonclinical staff. Rainbow Hospice Care also has its own freestanding inpatient facility in Johnson Creek, a location that's easily accessible to our entire service area.

Rainbow Hospice serves patients with any type of life-limiting illness such as cancer; heart, lung, and liver diseases; stroke; Parkinson's disease; Alzheimer's; ALS; or AIDS; with a life expectancy of less than six months. Regardless of age, race, gender, faith, ethnicity, insurance coverage or economic status, we treat all of our patients and their loved ones with the same respect, kindness and concern, focusing on quality of life, maintaining comfort and dignity, and providing expert support tailored to each individual's unique needs and wishes.

Hospice services are generally covered by Medicare, Medicaid, or private insurance plans. When a patient is ineligible for coverage or otherwise uninsured, care is never denied due to inability to pay.

Home Care office is located at 147 W. Rockwell Street in Jefferson, WI. Our Inpatient Center is situated just north of the intersection of I-94 and Hwy 26 at 1225 Remmel Drive, Johnson Creek.
